Post Title: Senior Health & Wellbeing Practitioner (SHWP)

Work area: The Well Centre / Herne Hill Group Practice

Responsible to: The Clinical Director / Services Manager

Locations: The role will be primarily based at The Well Centre / Herne Hill Group Practice, out-reach work in Lambeth / Wandsworth boroughs to be expected

Contract: Permanent

Hours of work: 37.5 hours a week, to be worked with a willingness to be flexible. This post includes late afternoon / evening work, typically finishing at 7:00pm to provide after-school appointments for young people. (This is not a remote role)

External contacts: Young people and their families; Youth Clubs; project funders; Social Services; Heath Authorities; Education Authorities; Schools; Voluntary Sector.

The Well Centre is an innovative Youth Health Hub delivered by Herne Hill Group Practice which provides access to a range of health, information and advice services for young people aged between 11 and 20 years old. We provide services for young people from Lambeth & Wandsworth across our few locations in these two boroughs.

We want the young people who come to us to be ‘healthy, happy and safe’. We provide sessions with Health & Wellbeing Practitioners, Counsellor and GPs. We work on a holistic model of health in partnership with local agencies to ensure that young people’s needs are met.

The post holder will be a senior team member leading a team of Health & Wellbeing Practitioners (under 8 currently) providing them with support / advice / guidance.

You will work closely with the Clinical Director to ensure the HWPs are providing an adequate service, follow the correct procedures, ensuring quality of their work and achieving agreed standards. The post holder will be providing HWPs with monthly supervision, reviewing their caseload, offering advice, identifying HWP limitations, risk assessing the cases, escalating to more appropriate clinician/service as required.

The post holder will also carry their own caseload of young people to deliver a response to the unmet needs of young people through the implementation of a young person’s social prescribing model. The role will connect young people in to a broad range of services and interventions in their local area in a timely and effective way, led by the wants and needs of the young person’s themselves.

Job Description and main duties:

· Provide HWPs with monthly supervision meetings

· Carry out monthly caseloads audits ahead of supervisions to:
- identify gaps in support/recording/training
- ensure correct Emis procedures/codes are being used for reporting procedures
- advise when case needs escalating
- identify successful work & highlight this in supervisions

- ensure the outcome measurements are correctly recorded to evidence the effectiveness of the engagement with the HWPs

- ensure a written feedback has been sought from young person about their sessions and it is reflected upon

- ensure appropriate discharge letter has been produced and sent back to the referrer

· Plan and lead twice monthly HWP meeting to share learning/networking/new activities/communicate service developments

· Support the team to plan & implement group sessions

· Attend triage & monthly mental health consult meeting with the Counsellor

· Provide informal check-ins with HWPs about issues/gaps arising in their work

· Provide in-house training e.g. creative ways to engage young people (YPs), group work, group session planning. ‘What is youth work’ type training session in HWPs induction

· Review and develop HWP procedures according to changing needs of the team and YPs

· Assist in shortlisting, recruiting new HWPs as required and organise their induction training

· Shadow new HWPs 1-1s during induction/probation to ensure they are competent within their roles

· Provide support to HWPs caseload in times of staff sustained sick leave

· Request team to write case studies to evidence benefit of HWP support

· Carry out weekly review of incoming referrals, supporting Services Co-ordinators to respond appropriately/deciding when referrals are ineligible

· Represent The Well Centre at various events and meetings to promote the work of the service and its achievements

· Network with other organisations identified by self and managers to develop longer term, sustained and beneficial partnerships. Feed back to relevant team members.

· Manage budget for group/youth activities and plan out a yearly group activities curriculum so better planned and communicated across whole team

· Cover and support drop-in clinics in-house and at schools

· Manage own smaller caseload of YP, signpost/refer YP to services & liaise with YPs network as necessary

· Proactively contact, engage and inspire young people to participate assessing their needs and offering a personalised approach to include face to face meetings, home visits, telephone / video support, or social media as required.

· Motivate, empower and encourage young people to take positive action to improve their health and wellbeing, by connecting with others, attending groups, promoting self-care, volunteering, accessing advice and information and support services. Set goals and develop plans with young people to help them take control of their health and wellbeing.

· Work with young people in a supportive, holistic way to address practical and psychological barriers.

· Attend reflective practice and or clinical supervision, team meetings and line management meetings as required – be involved in the development of good practice.

· Support youth engagement to ensuring that they have opportunity to have a voice, get involved and influence operational and strategic direction of the service

· Ensure young people are made aware of key processes e.g. complaints, information sharing, getting involved, and to make sure they are asked about how they would prefer information; to ensure special information requirements (e.g. large print, language, braille etc.) are clearly recorded on their files and their needs are met.

· Work in accordance with all Well Centre’s policies and procedures and attend mandatory trainings.

· Work as a member of the Well Centre and Herne Hill Group Practice team and undertake any other reasonable duties as may be required from time to time and at all times to present and conduct yourself in a manner becoming to the post and the professional dignity of the organization.

Person Specification

Essential:

· Minimum 3 years working background in health / social care or working with vulnerable young people

· Experience of working in an urban multi-cultural area

· To be non-judgemental and to take positive approach to all young people regardless of their age, gender, trans status, marital status, pregnancy, race, ethnicity, disability, sexual orientation, medical condition, religion or belief

· Able to follow processes and systems when accepting referrals, assessing young people, developing action plans and following up

· Excellent communication skills, able to negotiate, build relationships, advocate for young people and inspire others

· To be sensitive to the needs of individuals and communities that are perceived as harder to engage

· The ability of effectively communicate with a wide range of stakeholders, including good social interactions and listening skills

· Driven, target focused and highly motivated

· Resilient and confident, able to work in a busy environment with colleagues under pressure and champion the service to other professionals

· Outgoing, energetic, and passionate about improving the health and wellbeing of young people

· Good IT skills and experience of using EMIS database (EMIS trading provided for the right candidate)

· Good knowledge of information governance and ability to maintain confidentiality at all times, within any statutory guidance on safeguarding

· Able to work autonomously and be a self-starter

· Able to take decisions and use professional expertise, but within a structured framework and existing systems and policies.

· Effective team player who contributes to the success of others.

· Experience of motivating, empowering, supporting people to achieve goals welcomed

Qualification / Training / Knowledge

· Youth work, Social work, Health or equivalent at degree level

· Supervisory experience (minimum 2 years)

· A sound understanding of Child Protection & Vulnerable Adult Safeguarding issues and procedures

· A knowledge of & ability to support appropriate pathways relating to physical, mental health and social care for vulnerable children and young people

· Ongoing commitment to learning in relevant areas such as safeguarding and related issues which may impact on young people such as education, housing, relationships and justice
Commitment to undertake mandatory training
