Housing and Accommodation Advisor for HPFT Inpatient Services
Job purpose:
The post-holder will work directly with service users in housing need who are patients on inpatient mental health wards. The aim of the role is to ensure wherever possible that when patients are medically fit for discharge, that this is not delayed due to housing issues. In addition, the role is to prevent readmissions due to housing needs.
