Job Details

Social Prescriber

We are looking for an experienced youth worker to specialise in promoting the emotional well-being of children and young people.

This will include creating or supporting personal plans that combine direct delivery, on-line tools and integrated support services. The role will be based across the three youth hubs (Lift, Platform and Rose Bowl).

There will be a period of training, programme and groundwork development prior to direct referrals starting in Autumn 2019.

This new role is part of an exciting new pilot programme that Isledon Arts CIC is running on behalf of  Islington Council and CCG.

Background:

  • An increasing number of children and young people with social, emotional and mental health issues are seeking professional help via their GP or school. In some cases the clinical assessment shows the young person would be better served in the first instance by the offer of a supported programme of social, physical and creative activities (social prescribing) alongside counselling or psychotherapy in a community setting.
  • Islington CCG in partnership with Islington Council are funding a one-year pilot programme to test a model to improve access to appropriate, non-clinical community support for adolescents (11-25 years) who are experiencing risks to their wellbeing. The Social Prescribing service offers an early intervention pathway for specific referrers in the borough (5 North London GP surgeries, a long-term conditions service and an educational setting) via a direct referral to a social prescriber.
  • Learning from the pilot will feed into the redesign of integrated social and emotional mental health services (SEMHS) in the Borough.
