Youth Worker - Adult Congenital Heart Disease

Location: Youth@Heart HQ, Gloucestershire & Bristol Heart Institute, Bristol

Salary: £23,023 - £29,608 pa pro rata

An exciting opportunity has arisen to work in collaboration with the Bristol Heart Institute in the Adult Congenital Heart Disease Service for a Youth@Heart charity funded youth worker/young adult support co-ordinator to support young people from age 16yrs to 25 years old.

This is a fixed term contract for 2 years or could be a secondment opportunity.

The adult congenital heart disease service for young people supports young people during their time as in-patients and out-patients and in the community from Bristol and the south west, Cardiff and south Wales.

Role responsibilities

Day to day activities will include,

  • Acting as a specialist resource to support the young adults with congenital heart disease teams across the South West and South Wales, supporting their families and advising staff on the unique needs of the young person with congenital heart disease.
  • Provide and lead a wide range of appropriate participatory activities to facilitate the personal, social and educational development of teenagers and young adults with congenital heart disease.
  • Be an excellent communicator and familiar with how electronic communication can help to support young people
  • Adapting approaches to support young people who may be particularly vulnerable.
  • Maintain a data base on activities, support and budget to facilitate on-going evaluation of the role.

Person Specification

The ideal candidate will have,

  • Experience in youth work age 16-25 yrs, ideally in a clinical and community setting
  • Proven ability in project work involving youth participation/engagement
  • Resource management skills
  • Be compassionate and able to relate well to people
  • Self-motivated and forward thinking
