Experienced Family Support Worker 

Salary range £30,012

Family Support is an independent organisation providing services to the local authority, London Borough of Hammersmith and Fulham.  Our independence allows us to be agile in the way we deliver our service and requires that our staff are agile and flexible. We are currently looking for a Family Practitioner, to provide a first-class service, supporting our Managing Director in his endeavours to ensure all children and families in Hammersmith are supported to achieve the best outcomes.

The successful candidate will report to a Family Support Manager.

You will be responsible for holding a caseload of 10-15 families with children and young people aged 0-19 years carrying out assessments and chairing Team Around the Family meetings (TAF). You may either be supporting these families as the lead professional or working jointly with Social Work colleagues.  Experience of systemic practice and Signs of Safety would be beneficial and use of Mosaic as a case file recording system would be a distinct advantage.  Direct work with children and young people is required every 2 weeks so successful candidates will be expected to manage their own hours to meet the needs of the clients.  Family Support maintains a robust monitoring procedure and in line with this, candidates will receive monthly supervision and case file auditing. Experience working as part of a multi-agency team and network.

Experience as a family practitioner desired​.
