Experienced Family Support Duty Practitioner
Salary range: £33,459 - £37,650
Interviews:TBC
Family Support is an independent organisation providing services to the local authority, London Borough of Hammersmith and Fulham. Our independence allows us to be agile in the way we deliver our service and requires that our staff are agile and flexible. We are currently looking for a Family Support Duty Practitioner, to provide a first-class service, supporting our Managing Director in his endeavours to ensure all children and families in Hammersmith are supported to achieve the best outcomes.
The successful candidate will report to the Director of Operations
Cases are sent to Family Support and are then allocated to named Family Support Practitioners. This process often involves cases being managed centrally for a period until a Practitioner becomes available. The Duty Practitioner role is to safely manage and prioritise these cases and then allocate to an appropriate named Practitioner as soon as possible.
You will be an experienced practitioner with an excellent grounding in safeguarding. You will be contacting families and assessing need and risk. You will be liaising with practitioner Teams and partner agencies. But most of all you will be able to focus on the needs of families. Experience of systemic practice and Signs of Safety would be beneficial and use of Mosaic as a case file recording system would be a distinct advantage. Experience working as part of a multi-agency team and network
This role requires an individual who is proactive, who can work independently and as part of a team. You will come into contact with our community and our families and so the ability to communicate to and across a spectrum of individuals is very important.
Together we have an ambitious plan to deliver more responsive and impactful early intervention that works in partnership with other organisations, the community and families. We want to see life chances enhanced and over ten years the cost (both human and financial) of failure reduced. We need proactive staff who are up for a challenge, willing to take responsibility and who share and want to be a part of our vision.
If you have the experience, drive and appropriate essential requirements, please do apply.
Company
We are here to help families in Hammersmith & Fulham thrive.
Our family centres are on hand so mums, dads, carers and children can make new friends and enjoy play and learning experiences together.
Loving, positive and nurturing relationships help children and young people develop the healthy brains they need for future life challenges.
And anything that gets in the way of these positive experiences can lead to toxic stress causing a child's healthy brain development to become impaired.
The good news is our brains aren't fully formed until around the age of 25, so there are plenty of opportunities to help young people develop the skills they need and to overcome the impact of earlier toxic stress.
Find out more about the science behind brain development.
Our family centres
Our family centres offer a range of experiences for families with children. These include learn and play sessions, baby massage, health visitor clinics, English language classes, sleep workshops and much more.
For older children and young people we offer access to youth provision and are here to help young carers. We also offer support to families who have special educational needs and disabilities.
All our experiences help to ensure that toxic stress is reduced for your child or young person and that they have the best start in life.

Here for families
Having children can be great fun but sometimes things can get hard and we are here for worries big and small. It could be dealing with problems at home with drugs or substance abuse, struggling to cope or feeling low. Perhaps you have housing problems or need help keeping the kids active or eating well.
We are here to provide quick and easy access to support and advice.
We help families thrive by building their confidence so that they can resolve issues before they become more difficult to manage. And that means we can help children and young people develop the resilient brains they need for future life challenges.
