The Children and Young People Disability Service 0-25 service is part of MOSAIC, the integrated service for disabled children who have complex developmental needs. Within this challenging role you will focus on assessment and care planning of children and young people with severe to profound disabilities and complex health needs from birth to 25. You will be working with complex families in a multi-disciplinary and multi-agency setting with health practitioners and work closely with special educational needs and learning disabilities adults colleagues with a focus on Preparing for Adulthood.

The role

We are looking for a super talented, dedicated, highly motivated Senior Practitioner who is keen to progress and share our values of working as one team and finding better and more innovative ways of doing things. You will need to be a pro-active analytical thinker, with experience of working in an integrated setting with disabled children and young people, experience of preparing for adulthood work and experience of safeguarding and an understanding of the Children and Families Act 2014, Care Act 2014 and Preparing for Adulthood agenda. You will also need to be a strong team worker, who brings ideas for service delivery and in partnership with parents and young people.

About you

To succeed you must hold a qualification in Social Work, CQSW or equivalent with a minimum 2 years post-graduation experience, registered with HCPC. Demonstrable previous experience of assessments, safeguarding disabled children and working in a multi-disciplinary / agency setting are essential. The ideal candidate will possess a good understanding of childcare legislation relevant to this post; demonstrate excellent negotiation and influencing skills and ability to deal with conflict, hostility and vulnerability.

The successful candidate will be able to demonstrate balancing competing needs of supporting families, but remaining child focused and ensuring disabled child's needs/safeguarding issues are addressed, whilst managing a dynamic, complex and diverse caseload.  This role can be challenging at times but truly rewarding.
