The Integrated Care Pathways Team works with young people aged 16 plus with particular focus on vulnerable young people who face multiple barriers including obtaining and sustaining Employment, Education or Training. There are opportunities available for motivated, skilled and experienced practitioners who are committed to ensuring vulnerable young people receive the support and direction, to be safe, healthy and to progress as independent, resilient young adults.
Lead professional Team Leader
PO2 £30,987 - £33,510 per annum inclusive of London Allowance

We are seeking a committed and experienced team Leader who has substantive experience of managing teams as well as extensive experience of working with vulnerable young people to improve outcomes including Education, Training and Employment. You will be highly motivated, able to lead and support a team of practitioners to ensure service is delivered to the highest standards. You will be effective and have a good knowledge of Quality Assurance and performance management including tracking and detailed data analysis to inform needs analysis and service planning. Having worked with Integrated Services you will bring along a range of effective partnership experience to ensure that young people, particularly those who are vulnerable and hardest to reach are provided with a responsive, effective, high quality provision.
The Post will include some evening and weekend working
